BACKGROUND:Knowledgeofthevariablerelationofthepersistentmedianartery(PMA)tothemediannerveanditscontributiontotheformationofthesuperficialpalmararchisofgreatclinicalsignificance.ThisstudypresentsaproposalofspecificvariableswhichmightbeintroducedtocharacterizethePMAinthewristregion.

MATERIALANDMETHODS:Onehundredandtwenty-fiverandomlyselected,isolatedupperlimbsfixedin10%formalinsolutionweresubjectedtoanatomicaldissection.

RESULTS:Ofthe125upperlimbs,PMAwasfoundin5specimens(4%ofthetotalnumberoflimbs).Inthecarpaltunnel,thearteryoccupiedtheanterolateralposition(2cases),theanteriorposition(2cases)ortheanteromedialposition(1case)inrelationtothemediannerve.TwotypesofsuperficialpalmararcheswithsignificantcontributionsfromthePMAwereobservedinthestudiedmaterial:completemedio-ulnararchandanincompletearchwithoutaconnectionbetweentheterritoriesoftheulnarandmedianarteries.ThemeanratioofthediameterofthePMAtothediameterofulnararteryatthelevelofthewristwas0.59(min.=0.38,max=0.83,SD=0.19).

CONCLUSIONS:OrthopedicandhandsurgeonsshouldbeawareoftheprobabilityofoccurrenceofthePMAinbothplanningandconductingsurgerieswithinthewristregionandwithinthecarpaltunnel,asthisanomalousvesselmightpresentsignificantcontributionstothearterialbloodsupplyofthehandandmightpotentiallyplayanimportantroleinthepresenceofnotableclinicalsymptomsandpresentations.
